Ticket Sync

Ticket sync connects the ADLC ticket store to GitHub Issues. Pull imports issues as tickets, push updates existing issues, creates issues for tickets that exist only locally and writes execution metadata into the issue body, and sync runs both in turn. Everything is a preview until you pass the write flag, and all network calls go through the gh command line. A doctor command runs read-only offline checks on the setup, tickets and sync state.
