Human in the loop
Requiring a person to approve a specific class of action before an agent takes it.
The control that makes everything else survivable, and the one that decays fastest. A confirmation asked once is a decision. The same confirmation asked forty times a day is a keystroke.
So the useful version is selective: approval on the actions that are irreversible, cost money or leave the building, and no approval at all on the rest. An agent that asks about everything trains the person to approve everything.
It is the difference between an agent that can make a mistake and an agent that can make an unrecoverable one.
Measuring it by whether approval exists rather than by whether anybody reads it. Count how many approvals were refused last month; if the answer is zero, there is no loop.
